All youth 7th-12th grade must have on file with the Youth Pastor, a completed Health Form, good for the current calendar year to participate in any off- campus activity involving transportation away from church.  Each youth must also complete and return the current Permission Form for that event (to be updated for each event).  Just click on the link below, download the appropriate needed form, complete it, have it sign by a parent or guardian, and return it to the Youth Pastor, by the Deadline of that event.  (Permission Form to be added soon.  Keep checking.)
